1.   Please download and print this Adobe PDF form.  Bills Casino Credit Application
2.   Fill in all applicable blanks completely, being sure to include:

  • the amount of credit you are requesting
  • your complete bank(s) information with account number(s)
  • your date of birth and social security number
  • your signature and date

3.   Please note for your application to be considered you must also include a copy of your driver's license and a voided check along with your application.

4.   Once your application is completed, signed and dated, please return it to us with the copies of your drivers license and a voided check using one of the following options:
